Responsibilities
- Develops & maintains a clinical practice in the oncology setting, cooperating with medical & nursing personnel to optimize the pharmacotherapeutic aspects of patient care.
- Provides optimal selection of pharmacologic agents and design of a regimen to maximize therapeutics benefit and minimize toxicity.
- Provides daily follow-up on the clinical effects of the regimen with adjustments as necessary to achieve desired outcomes.
- Provides drug and clinical information to other healthcare professionals in response to specific questions relating to new evolving issues related to the care of patients with cancer.
- Provides professional education regarding new drugs, biologics or technologies for specialized drug delivery.
- Provides a significant level of education to professionals, trainees, residents, students and patients.
- Responsible for monitoring and assessing performance of the learner and providing feedback using appropriate learning objectives that are aligned with the desired learning outcomes.
- Performs pharmacokinetic dosing and monitoring for all patients on drugs where therapeutic levels are reported.
- Provides visual, spoken and written clinical education and training to pharmacy students, pharmacists, clinical pharmacists and residents in the Department of Pharmacy.
- Provides managerial responsibilities for the operations of the Cancer Center satellite, including process improvements and maintaining oncology formulary.
- Designs, supports and implements policies to provide effective and efficient use of resources for ensuring that budget, quality and value targets are met in collaboration with the Acute Care Manager.
- Develop, review, assist and maintain, in collaboration with members of the Department of Hematology/Oncology various protocols and regimens within the computerized physician order system.
Requirements
- Bachelor of Science degree in Pharmacy from an approved college of Pharmacy required.
- Doctor of Pharmacy (PharmD) degree preferred.
- Registered as a licensed pharmacist or eligible for reciprocity in the State of Connecticut. (Must obtain a registered license within 3 months of employment).
- Board Certification in Oncology required within 1 year of employment.
- Previous oncology pharmacy practice experience (3 to 5 years) in institutional environment and/or completion of an ASHP-accredited hospital (PGYI & PGY2) residency with a focus in oncology.
- Member of the local, regional, and national professional organizations preferred.
- Demonstrated knowledge and compliance with all State, Federal, and malpractice laws of pharmacy practice.
- Excellent communication and interpersonal skills and experience working on interdisciplinary initiatives across all levels of organization.

$59.01 - $109.58 per hour
