Responsibilities
- Participate and lead pharmacy clinical/operational programs
- Monitor patient medication regimens and provide appropriate interventions
- Review, interpret, and input treatment orders to ensure accuracy
- Support service lines through drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing, and other programs
- Provide and support clinical pharmacy responsibilities by collaborating with other hospital disciplines
- Collaborate with medical and nursing staff to optimize pharmacotherapeutic aspects of patient care
- Provide medication and practice-related education and counseling to patients, caregivers, and medical staff
- Oversee the preparation of treatments, preparation of investigational treatments, prepackaging of medications, and drug returns
- Maintain records on patient prescriptions, charges, and inventory
- Ensure pharmacy technician duties are performed completely, safely, and without risk of harm to patients
- Analyze, compare, and evaluate various courses of action; make independent decisions
- Manage stem cell transplant and cellular therapy therapies, order sets, and REMS requirements
- Provide staff education regarding cellular therapy management
- Directly support stem cell transplant and cellular therapy patients
- Aid in the precepting of pharmacy students and residents
- Perform all other duties as assigned
Qualifications
- Knowledge of and compliance with state and federal pharmacy laws and regulations
- Excellent communication skills
- Organization and problem-solving skills
- Ability to align patient care activities and project work with departmental and institutional policies, business model/needs, goals, and vision
- Thorough understanding of dosage administration and measurement, chemical compounds, medical brands, etc.
- Education: PharmD required or equivalent
- Certification/Licensure: Texas Pharmacy license required at time of hire; BCOP strongly preferred
