About the role

Optimizes patient care by promoting rational, safe, and cost-effective drug therapy through clinical services in their area of expertise. Conducts drug therapy monitoring and formulary reviews, implements interventions to reduce adverse events, educates and trains pharmacy staff, residents, students, and healthcare providers.

Responsibilities

• Reviews and evaluates patients' drug/medical histories to provide drug therapy recommendations.
• Provides drug information, evaluates medical literature, rounds with medical services, conducts drug utilization reviews, monitors, and patient counseling.
• Plans, implements, and measures clinical processes to improve treatment outcomes and minimize adverse drug events, contributing to favorable quality and financial outcomes.
• Collaborates with healthcare teams to develop and implement clinical pathways and protocols promoting medication safety and utilization.
• Trains and coaches pharmacists, pharmacy residents, students, and healthcare providers on clinical processes.
• Contributes to the medical community by presenting research findings at meetings and publishing results in journals.
• Performs other duties as assigned or required.

Qualifications

• Minimum Pharmacy Residency (PGY1) or 4 years of demonstrated clinical experience.
• Licensed or eligible for licensure as a Registered Pharmacist in the state of PA or eligible to obtain licensure within 120 days.
• Preferred PGY2-Residency.
