About the role
Optimizes patient care by promoting rational, safe, and cost-effective drug therapy through clinical services in their area of expertise.
Responsibilities
- Reviews and evaluates patients' drug/medical history to provide drug therapy recommendations.
- Provides drug information, evaluates medical literature, rounds with medical services, and conducts drug utilization reviews/monitors.
- Counsels patients on medication use.
- Plans, implements, and measures clinical processes to improve treatment outcomes and minimize adverse drug events.
- Analyzes, trends, and evaluates data to synthesize reports for committee action.
- Develops and implements clinical pathways and protocols to promote medication safety and utilization.
- Trains pharmacy staff, residents, students, and healthcare providers on clinical processes.
- Contributes to the medical community by presenting research and publishing results.
Qualifications
- Minimum Pharmacy Residency (PGY1) or 4 years of demonstrated clinical experience.
- Licensed or eligible for licensure as a Registered Pharmacist in the state of PA or eligible to obtain licensure within 120 days.
