Duties and Responsibilities
- Participates in daily patient care rounds.
- Makes sure drug therapy is appropriate, correct doses, forms, regimens, routes, and drug interactions are checked.
- Communicates with doctors about medication orders, updates patient records.
- Provides care in critical care areas (CCU, ICU).
- Maintains and updates patient profiles daily.
- Promotes use of the formulary by converting non-formulary orders to formulary when possible.
- Assists with medication-related inquiries from other health care professionals and patients.
- Documents clinical interventions and cost avoidance.
- Serves as a consultant to hospital Pharmacy & Therapeutics committee and other clinical committees.
- Contributes to newsletters, in-services, and educational materials.
- Plans and implements system-wide pharmaceutical care programs.
- Reports daily to Clinical Pharmacy Manager.
- Responds to medical emergencies.
- Counsels patients on discharge medications and coordinates meds to beds and interventions with Outpatient Pharmacy.
Job Specifications
- Required Knowledge, Skills and Abilities:
- Knowledge: Pharm.D. or equivalent from an ACPE-accredited School of Pharmacy and a current pharmacist license in good standing with appropriate State Board of Pharmacy. Critical Care PGY1,2 residency recommended or at least 3 years appropriate clinical experience is recommended. Board Certification Recommended.
- Experience: Using computer applications including word processing, spreadsheets, and pharmacy information systems.
- Skills: Thorough knowledge of state and federal laws and regulations governing pharmacy services. Proficient in the use and application of new medications and technology. Effective verbal and written communication skills are required. Ability to effectively participate on an interdisciplinary team or committee. Knowledge of pathophysiology, interpretation of diagnostic criteria, pharmacokinetics and pharmacotherapeutics. Problems solving ability and analytical thinking preferred.
- Abilities: Ability to work independently and handle stress appropriately. Ability to work cooperatively with other members of Insight Hospital and Medical Center and pharmacy staff. Ability to handle multiple tasks and adapt to changes in workload and work schedule. Ability to set priorities, make critical decisions, and respond to client concerns. Fosters a team environment. Ability to communicate effectively (verbal and written).
