Clinical Pharmacy Specialist

About the role

Join a progressive Ambulatory Care pharmacy team at Community Health Network. Participate in team-based care under collaborative drug therapy agreements. Manage hypertension, dyslipidemia, cardiovascular risk reduction, antiarrhythmic monitoring, tobacco cessation, anticoagulation service, and transitional care management for cardiology discharge patients both in-person and virtually.

Responsibilities

• Manage hypertension, dyslipidemia, cardiovascular risk reduction, antiarrhythmic monitoring, tobacco cessation, anticoagulation service, and transitional care management for cardiology discharge patients both in-person and virtually.
• Provide staff education, guideline/policy development and implementation, and participation in performance improvement initiatives and practice advancement.
• Mentor and teach pharmacy students and residents.
• Serve as a clinical resource for the healthcare team.
• Engage in population health projects to impact the lives of patients we serve.
• Have a clear understanding of quality improvement principles.

Requirements

• Strong communication skills and a collaborative approach to patient care.
• PRACTICE AT THE TOP OF YOUR LICENSE utilizing established and growing collaborative drug therapy management agreements.
• MEET WITH PATIENTS INDIVIDUALLY (IN-PERSON AND REMOTELY) to provide extensive drug and disease state education with a particular focus on patients at high risk for hospitalization or poor health outcome.
• WORK CLOSER WITH PHYSICIANS AND HEALTH CARE TEAMS, while providing direct patient care.
• WORK CLOSER WITH PATIENTS TO OVERCOME MEDICATION ADHERENCE ISSUES.
• AID PATIENTS IN OBTAINING COST EFFECTIVE MEDICATIONS with the assistance of medication assistance coordinators.
• ASSIST IN MONITORING FOR DRUG-THERAPY-RELATED ADVERSE EVENTS by performing/ordering the appropriate clinical or laboratory tests.
• ENGAGE IN POPULATION HEALTH PROJECTS to impact the lives of patients we serve.

Qualifications

• Graduate of an accredited college of pharmacy, Bachelor or PharmD; Doctorate degree.
• Indiana Pharmacist license.
• Successful completion of a PGY1 Pharmacy Residency and PGY2 residency with Cardiology or Ambulatory Care focus is strongly encouraged or a minimum of three (3) years working under a drug therapy agreement in a clinical setting.
• Board certification as a Board Certified Pharmacotherapy Specialist, Board Certified Ambulatory Care Pharmacist, Board Certified Cardiology Pharmacist is encouraged and required within 2 years of accepting the position.
