Clinical Pharmacy Specialist

Join our Shields team at a nationally recognized Academic Medical Center, where you’ll play a hands-on role in elevating patient care through health-system integrated specialty pharmacy.

About the role

In this dynamic, patient-centered position, you’ll build meaningful relationships with patients and providers while delivering high-quality medication management. You’ll work on-site in clinic, collaborating directly with care teams and practicing under a Collaborative Practice Agreement (CPA) to conduct in-person and virtual patient assessments, order and monitor evidence-based treatment plans, optimize therapy, and resolve medication-related problems.

Location: Onsite at Providence Medford Medical Center in Medford, OR

Responsibilities

• Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments aligned with evidence-based standards.
• Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination.
• Monitor therapy effectiveness and safety, identify optimization opportunities, and intervene as appropriate.
• Support quality-improvement and REMS initiatives.
• Provide recommendations to operations by supporting teammates as needed with complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.

Requirements

• Hold a Doctor of Pharmacy (PharmD)
• Must be a licensed pharmacist in Oregon
• Candidate must meet one of the following qualifications:
  • Completion of a PGY2 Ambulatory Care Residency
  • Completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA)
  • At least 3 years of ambulatory practice with disease state management
• Strong communication, organization, and tech skills
• Empathy, integrity, and a team-first mindset
• Proficiency in Microsoft Office tools (Outlook, Teams, Word, Excel, and PowerPoint) for communication, documentation, and data management, as well as navigating the internet and web-based platforms

Data entry and computer skills will be evaluated through an assessment as part of the pre-employment process.

Pay

An employee in this position who performs the job duties in Oregon can expect a salary rate between $140,000 and $170,000 (Hourly Equivalent: Approximately $62.50 - $72.12 per hour, based on a 40-hour workweek) plus bonus pursuant to the terms of any bonus plan if applicable. The actual salary will depend on experience, geographic location, and other factors permitted by law.
