Responsibilities

  • Interprets orders and transcribes or verifies them accurately and efficiently into the pharmacy computer system
  • Accurately compounds and dispenses pharmaceuticals
  • Complies with facility policy and procedures and regulatory requirements
  • Maintains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promoting rational drug therapy
  • Sustains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promoting rational drug therapy
  • Supports participation in daily multidisciplinary and bedside rounds (may need to attend)
  • Identifies, designs and analyzes drug utilization reviews to promote rational drug therapy and determines action plan based on results
  • Develops, manages and mentors for the antimicrobial management program (AMP)
  • Develops, manages and mentors for the antimicrobial management program (AMP)
  • Develops and actively participates in training and mentorship programs for pharmacists, pharmacy students, residents, technicians, nurses and physicians
  • Develops and documents staff competency on an ongoing basis and develops improvement plans when deficiencies in competencies are identified
  • Primary preceptor for pharmacy students and/or residents as it relates to clinical pharmacy services
  • Develops and manages specialty pharmacy practice programs related to clinical pharmacy

Requirements

  • Pharmacist
  • Bachelor's Degree, or Doctorate Degree
