Responsibilities
- Interprets orders and transcribes or verifies them accurately and efficiently into the pharmacy computer system
- Accurately compounds and dispenses pharmaceuticals
- Complies with facility policy and procedures and regulatory requirements
- Maintains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promoting rational drug therapy
- Sustains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promoting rational drug therapy
- Supports participation in daily multidisciplinary and bedside rounds (may need to attend)
- Identifies, designs and analyzes drug utilization reviews to promote rational drug therapy and determines action plan based on results
- Develops, manages and mentors for the antimicrobial management program (AMP)
- Develops, manages and mentors for the antimicrobial management program (AMP)
- Develops and actively participates in training and mentorship programs for pharmacists, pharmacy students, residents, technicians, nurses and physicians
- Develops and documents staff competency on an ongoing basis and develops improvement plans when deficiencies in competencies are identified
- Primary preceptor for pharmacy students and/or residents as it relates to clinical pharmacy services
- Develops and manages specialty pharmacy practice programs related to clinical pharmacy
Requirements
- Pharmacist
- Bachelor's Degree, or Doctorate Degree
Qualifications
