Clinical Pharmacist - Surgical Intensive Care and General Surgery

Position Highlights

Our Clinical Pharmacists provide extensive pharmaceutical care and are a vital part of the inpatient multidisciplinary patient care team. In addition to direct patient care responsibilities, our pharmacists provide drug therapy consults, participate in treatment protocol development, provide patient/family/staff education, facilitate transitions of care and precept pharmacy students and residents.

Work Schedule

Full-Time / 40 Hours Per Week, Rotating/Variable Shift.

Responsibilities

• Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies.
• Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.
• Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations.
• Provide drug information and education to medical teams, nursing, and dietitian staff.
• Provide patient/family counseling and education.
• Update patient medication lists and reconcile medication orders at each visit.
• Provide primary preceptorship of pharmacy residents and student pharmacists.
• Perform duties of a Pharmacist 2.

Qualifications

• Two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.
• State of Washington Pharmacist License (legal requirement).
• Applicants should have an advanced degree, or residency, or two years of experience in clinically-oriented hospital pharmacy practice. PGY-2 critical care training preferred.

Compensation

Pay Range: $154,752.00 – $221,436.00 annually.

Shift: Alternate Work Shift.

This is a regular position at 100% FTE.
