Clinical Pharmacist

Responsibilities

• Maintains medication usage for participants ensuring safety, efficacy, cost efficiency, and optimal outcomes.
• Serves as a drug information resource for medical and nursing teams, participants, and their families.
• Evaluates medication regimens upon intake and periodically, addressing drug interactions, disease states, pharmacokinetics, and food.
• Reviews participants discussed at IDT meetings for medication-related issues and acts as a resource for interdisciplinary team members and clinic staff.
• Aids primary care team in optimizing drug therapy outcomes by reviewing aggregated drug-usage reports and performing drug utilization reviews.
• Collaborates with primary care physicians and IDT team to remove unnecessary prescriptions and adjust drug therapy based on participant goals.
• Monitors participant responses to drug therapy and optimizes treatment plans based on disease states, standards of care, and patient goals.
• Assesses dementia patients on psychotropic medications for potential dose reductions and ensures adequate drug monitoring.
• Conducts interviews and consultations with participants and/or caregivers to assess medication efficiency and address any concerns.
• Provides educational sessions to participants and caregivers about medication effects, side effects, and proper usage.
• Supports primary care team in enhancing drug therapy outcomes by offering education and participating in quality-initiated projects.
• Involves in developing and implementing departmental policies and procedures, and participates in team-building and development initiatives.
• Maintains awareness of new drug regulations and therapeutic warnings, and ensures staff understands and follows these guidelines.
• Presents relevant topics on drug therapy in the geriatric population to IDT and Center staff.
• Solves medication use or access issues, analyzes operating reports, and takes corrective actions when necessary.

Requirements

• Graduate of an accredited school of pharmacy with Board Certification in Geriatric Pharmacy (BCGP) or Pharm D.
• Current State Board of Pharmacy license for the state of practice.
• Knowledge and ability to advise on medication uses, side effects, and potential problems to physicians.
• Obtain BCGP certification within 30 months of hire if not already certified.

Preferred

• New Mexico Pharmacist Clinician License.
