Clinical pharmacist
About the role
The inpatient pharmacy at Trinity Health Ann Arbor is open 24/7, serving inpatients, the emergency department, operating rooms, and multiple procedural areas. The pharmacy is accredited by Joint Commission for adherence to USP 797 and 800 standards.
Responsibilities
- Interprets and evaluates medication orders to ensure appropriateness of therapy.
- Maintains patient medication regimens, addressing allergies, drug interactions, and adverse effects.
- Individualizes medication therapy based on patient variables and consults with physicians and other healthcare providers.
- Evaluates and recommends patient-specific drug therapy and collaborates with medical staff.
- Provides patient education and counseling, participates in therapeutic drug monitoring, and educates healthcare professionals.
- Coordinates pharmaceutical services for hospital units and participates in emergency codes and responses.
- Directs and monitors pharmacy technicians, interns, and students in medication preparation and compounding.
- Participates in staff development programs, continuing education, and teaching programs for students and residents.
Requirements
A Bachelor's degree in Pharmacy is required, with a Doctor of Pharmacy degree or equivalent clinical degree preferred. Previous related hospital experience or residency is strongly preferred. A Michigan Pharmacist and Controlled Substance license is required, along with maintaining State Board of Pharmacy continuing education credits.
