Clinical Pharmacist Specialist – Float

We are seeking a Clinical Pharmacist Specialist / Float to deliver advanced clinical pharmacy services across multiple hospital units and specialties. In this collaborative, patient-centered role, you will optimize medication therapyperform order verificationconduct therapeutic drug monitoringmanage medication reconciliationand participate in patient rounding with interdisciplinary teams. You will also support evidence-based protocols, provide staff and patient education, and promote medication safety. The role involves floating to areas of highest need within the health system to ensure comprehensive pharmaceutical care.
