Clinical Pharmacist Specialist – Float

Responsibilities

• Provide advanced clinical pharmacy services across multiple hospital units and specialties.
• Optimize medication therapy and perform order verification, therapeutic drug monitoring, and medication reconciliation.
• Collaborate with interdisciplinary teams during patient rounds.
• Support evidence-based protocols and promote medication safety.
• Educate staff and patients to ensure proper medication use.
• Float to areas of highest need within the health system to deliver consistent pharmacy coverage.
