About the Role

The Clinical Pharmacist Specialist is responsible for the provision of safe, effective, appropriate, and cost-efficient medication therapy management to patients in Ambulatory Care. Under the direction of the Director of Pharmacy and site-based Medical Director, this role assists in planning, organizing, and monitoring the delivery of ambulatory care pharmacy services for Alameda Health System. The specialist coordinates pharmacy services, provides direct patient care, and delivers complex clinical services, including consultation for staff and clients.

This position primarily backfills Clinical Pharmacists across various Ambulatory Care Clinics, including Oncology, HIV, Substance Use Disorder (SUD), Heart Failure, Diabetes, Hypertension, Anticoagulation, Refill, and others. There is an opportunity to learn new clinics and services. Precepting students and residents is part of the role, and some clinics may allow optional remote work for certain shifts.

Responsibilities

• Works with the System Director of Pharmacy to coordinate, improve, expand, and deliver ambulatory care pharmacy services within Alameda Health System.
• Monitors patients in specific units or specialty services, including collecting patient-specific data, interpreting drug-related laboratory data, performing relevant physical assessments, interviewing patients, and identifying patient goals of therapy.
• Assesses patient-specific medical problems, prioritizes them based on urgency and severity, and identifies preventive and health maintenance issues.
• Evaluates patient-specific drug therapy for appropriateness, effectiveness, and potential interactions or adverse effects.
• Reports and documents drug errors, adverse drug reactions, allergies, and patient adherence issues.
• Designs comprehensive drug therapy and patient education plans, justifying recommendations based on pharmacologic, pharmacokinetic, pharmacodynamic, pharmacogenomic, pharmacoeconomic, ethical, legal, and evidence-based information.
• Under protocol, initiates, adjusts, monitors, and recommends changes in drug therapy based on patient interviews, objective monitoring parameters, physical assessments, medication profiles, and patient medical records.
• Documents clinical interventions in the electronic health record in a timely and professional manner.
• Collaborates with patients, caregivers, and other healthcare professionals.
• Develops and maintains the drug formulary for Alameda Health System and provides education to physicians and other healthcare providers regarding formulary additions and revisions, including non-formulary request reviews.
• Prepares drug monographs and cost analyses for presentation to committees, staff, physicians, and other stakeholders.
• Maintains knowledge of current evidence-based practices for applicable chronic disease states and serves as a drug information resource for healthcare providers and team members.
• Represents Clinical Pharmacy Services on various committees, staff meetings, and professional meetings related to the pharmacy area of practice or specialty.
• Coordinates continuing education activities for medical and nursing staff, orients new employees on pharmacy practices and procedures, and contributes to pharmacy newsletters, journal clubs, and in-services for professional staff.
• Serves as a preceptor to pharmacy residents, students, and volunteers.

Requirements

• Doctor of Pharmacy degree.
• Completion of an American Society of Health-System Pharmacy (ASHP) accredited Postgraduate Year One (PGY1) Residency.
• Postgraduate Year One (PGY1) or Postgraduate Year Two (PGY2) Residency in Ambulatory Care is preferred.
• Three to five years of recent ambulatory care pharmacy experience preferred.
• Possession of a valid registered pharmacist license to practice in California.
• Basic Life Support (BLS) certification required.
• Board certification (BCPS or BCACP) is preferred.
• National Provider Identifier (NPI) must be obtained upon hire.

Pay

Starting pay: $115.29 per hour. The pay range reflects the base pay scale for this role at Alameda Health System. Final compensation will be determined based on experience, education, skills, licensure, certifications, departmental equity, applicable collective bargaining agreements, and operational needs.
