About the Role

The Clinical Pharmacist Specialist is responsible for the provision of safe, effective, appropriate, and cost-efficient medication therapy management to patients in Ambulatory Care. Under the direction of the Director of Pharmacy and site-based Medical Director, they assist in planning, organizing, and monitoring the delivery of ambulatory care pharmacy services for Alameda Health System (AHS). This role coordinates pharmacy services within AHS and its clients, providing advanced clinical services and direct patient care.

This position supports Specialty Clinics on the K7 floor of Highland Hospital, starting with Neurology, Dermatology, and chronic disease management (e.g., Diabetes or Hypertension). Adaptability and problem-solving skills are important when working with multiple specialty clinics. Minimal remote work may be possible for telemedicine visits.

Responsibilities

  • Work with the System Director of Pharmacy to coordinate, improve, expand, and deliver ambulatory care pharmacy services within AHS.
  • Monitor patients in a specific unit/specialty service, including collecting patient-specific data, interpreting drug-related laboratory data, performing relevant physical assessments, interviewing patients, and identifying patient goals of therapy.
  • Assess patient-specific medical problems, prioritize based on urgency and severity, and identify preventive and health maintenance issues.
  • Evaluate patient-specific drug therapy by assessing appropriateness, effectiveness, and identifying potential interactions or adverse effects.
  • Report and document drug errors, adverse drug reactions, allergies, and patient adherence issues.
  • Design comprehensive drug therapy and patient education plans, justifying recommendations based on pharmacologic, pharmacokinetic, pharmacodynamic, pharmacogenomic, pharmacoeconomic, ethical, legal, and evidence-based information.
  • Under protocol, initiate, adjust, monitor, and recommend changes in drug therapy based on patient interviews, objective monitoring parameters, physical assessments, medication profiles, and medical records.
  • Document clinical interventions in the electronic health record in a timely and professional manner.
  • Collaborate with patients, caregivers, and other healthcare professionals.
  • Develop and maintain the AHS drug formulary, providing education to physicians and healthcare providers on formulary additions and revisions, including non-formulary request reviews.
  • Prepare drug monographs and cost analyses for presentation to committees, staff, and physicians.
  • Maintain knowledge of current evidence-based practice for applicable chronic disease states and serve as a drug information resource for healthcare providers and team members.
  • Represent Clinical Pharmacy Services on various committees, staff meetings, and professional meetings as needed.
  • Coordinate continuing education activities for medical and nursing staff, orient new employees on pharmacy practices, and contribute to pharmacy newsletters, journal clubs, and in-services.
  • Serve as a preceptor to pharmacy residents, students, and volunteers.

Requirements

  • Doctor of Pharmacy degree.
  • Completion of an American Society of Health-System Pharmacy (ASHP) accredited Postgraduate Year One (PGY1) Residency.
  • Postgraduate Year Two (PGY2) Residency in Ambulatory Care is preferred.
  • Three to five years of recent ambulatory care pharmacy experience preferred.
  • Valid registered pharmacist license to practice in California.
  • Basic Life Support (BLS) certification required.
  • Board certification (BCPS or BCACP) is preferred.
  • National Provider Identifier (NPI) must be obtained upon hire.

Pay

Pay range: $98.66 - $119.92 per hour. Final compensation will be determined based on experience, education, skills, licensure, certifications, departmental equity, applicable collective bargaining agreements, and operational needs.
