About the role

The Vancouver Clinic is seeking a Clinical Pharmacist to join their growing practice. The new clinician will work alongside physicians and advanced practice clinicians in a patient-centered environment. Responsibilities include optimizing medication regimens for patients with chronic or high-risk medical conditions, providing direct patient care, and participating in population health initiatives.

Responsibilities

  • Perform all necessary functions to ensure optimal use of medications to improve patient quality of life.
  • Serve as a clinical lead in population health and contribute to value-based care goals.
  • Manage the pharmacotherapy of complex patients and collaborate with physicians on collaborative drug therapy agreements.
  • See up to 15 patients per day through in-person and telehealth appointments.

Requirements

Preference will be given to candidates with residency training or 3+ years of experience. Newer ambulatory-trained pharmacists with quick learning abilities and excellent interpersonal skills are also encouraged to apply.

Qualifications

  • Full-time position (1.0 FTE).
  • Located in clinic offices in Vancouver and Portland with no weekend, evening, or holiday hours.
  • Active Washington license (preferred).

  • Salary model based on years of experience ($150,318 – $175,239).
