Clinical Pharmacist Specialist - Amb Care

Barnes-Jewish Hospital is seeking a Clinical Pharmacist Specialist to support a newly developed outpatient clinical service, collaborating with an interdisciplinary care team to optimize medication therapy, provide comprehensive disease state management, and improve access to specialty medications.

About the role

This role offers the opportunity to deliver direct patient care, support program development and quality initiatives, serve as a clinical resource for providers and patients, and contribute to the growth of ambulatory pharmacy services within a world-renowned academic medical center. PGY2 Residency preferred. Schedule includes Monday through Friday with 3 weekends per year.

The pharmacy at Barnes-Jewish uses an integrated practice model with unit-based positions and provides access to numerous drug information resources, including online references and an on-site drug information center for in-depth support.

Role Purpose

Evaluates and recommends appropriate drug therapy to achieve optimal cost-effective patient outcomes by working directly with physicians, other health professionals, and patients. Participates in the development, provision, and evaluation of comprehensive pharmaceutical care and quality assurance programs through scholarly activities. Serves as a resource to the pharmacy staff and other hospital departments.

Responsibilities

• Assess the overall status of the patient’s health condition and evaluate medication profile for optimal patient outcomes.
• Monitor therapy and goals of care.
• Document all activities and communication as appropriate.
• Participate in professional development and research activities.
• Contribute to performance improvement, clinical effectiveness, drug use evaluation, and HSO/BJC committee activities.
• Participate in the education and training of pharmacy staff, residents, and students, and serve as a role model and mentor.
• Participate in the identification and implementation of targeted drug cost reduction programs.

Requirements

• Doctorate in Pharmacy
• Less than 2 years of experience
• Medication Therapy Services in Missouri (MTS) certification

Preferred Qualifications

• 2-5 years of experience
