About the role

The clinical pharmacist shall practice the profession of pharmacy with the utmost code of ethics demonstrating the organization’s mission and values. The clinical pharmacist is responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in which they serve. The clinical pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of his or her work. The clinical pharmacist serves as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. Adherence to all regulatory and medication safety guidelines are at the forefront of all actions and decisions made within the practice of the clinical pharmacist.

Responsibilities

  • Safe, effective, evidence-based, and cost-effective selection and utilization of medications
  • Provide advanced pharmaceutical care within a multidisciplinary rounding team
  • Serve as a patient advocate in medication-related decisions
  • Collaborate with healthcare professionals in a knowledgeable and professional manner
  • Ensure adherence to all regulatory and medication safety guidelines

Requirements

  • Pharmacist license
  • Bachelor’s Degree or Doctorate Degree in Pharmacy

Experience

  • 7 years of experience required
