A newly added 22,000-square-foot facility in Niles, Michigan, houses 43 exam rooms and offers coordinated health services from diagnostics to primary and specialty care, delivering care to patients and their families within their community.
About the Role
Provide daily patient-focused clinical pharmacy services to inpatients and outpatients of all age groups within the Lakeland Regional Health System. Responsibilities include medication dosing, chart review, patient education, floor rounds, and physician intervention. Rotate through the distributive pharmacy as scheduled.
Responsibilities
- Perform order entry on the unit and in the pharmacy, fulfilling all functions of a staff pharmacist
- Complete medication reconciliation duties per policy
- Provide patient, nurse, community, and physician education
- Participate in the staff development education program
- Provide clinical training to Pharm.D. students as assigned
- Perform daily chart review, floor rounds, and physician intervention
- Provide long-term care nursing home consultation as assigned
- Participate in investigational drug protocols
- Perform accurate and timely input of interventions and adverse drug reactions (ADRs)
- Handle IV compounding and verification, adhering to quality control procedures
- Commit to continuous improvement of the patient experience, including patient safety, quality of care, and excellent service
- Adhere to the organization’s published values
- Perform other duties as assigned
Requirements
- Bachelor's Degree in Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
- 2 years of relevant experience in hospital pharmacy
Preferred Qualifications
- American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice
- PharmD and hospital experience (retail or new graduates may also be considered)
Licenses & Certifications
- State of Michigan Clinical Pharmacist License
- State of Michigan Pharmacist Controlled Substance License
- Basic Life Support (BLS) certification from the American Heart Association or American Red Cross

Schedule
Part-time, variable shift with 8 scheduled hours per week. Hours and days worked are variable, including variable weekends.
