Clinical Pharmacist PRN
About the role
Open availability and flexibility are required. This position consists of 8-hour shifts, with start times that may vary based on departmental needs. Shifts may begin as early as 6:00 AM or at 7:30 AM, 12:00 PM, or 1:30 PM (EST).
Responsibilities
- Education and counseling of patients, families, and healthcare team members on safe and effective medication use.
- Safe, timely, and cost-effective medication therapy; active participation in stewardship, surveillance, and restricted-use programs.
- Direct patient care and interdisciplinary rounds, ensuring smooth transitions of care from admission through discharge and follow-up.
- Supervision, mentoring, and evaluation of pharmacy technicians, interns, students, and residents; support team learning and development.
- Identification, reporting, and resolution of medication safety events, adverse drug reactions, and errors.
Requirements
- Bachelor's degree in Pharmacy (Doctorate preferred)
- One year of hospital experience strongly preferred
