About the role
The clinical pharmacist will be a part of MaineHealth’s Ambulatory Pharmacy Department, focusing on providing high-quality, direct patient care through collaborative practice and supporting primary care teams. The role involves seamless medication access and management for patients, with responsibilities including communication with patients and healthcare professionals, education, and collaboration with other pharmacists.
Responsibilities
- Work collaboratively with all healthcare professionals to provide safe and cost-effective patient care
- Communicate directly with patients and their families on all aspects related to medication therapy
- Promote high levels of knowledge, engagement, and adherence to medication therapy
- Ensure appropriate and optimized medication therapy for patients with chronic diseases
- Provide education to other healthcare professionals, including precepting trainees
- Partner with other healthcare professionals to develop evidence-based clinical pathways
- Generate and analyze process and outcome measures for use in assessing medication-related patient outcomes
- Demonstrate ongoing professional development by meeting continuing education requirements, staying updated on professional literature, and applying it to clinical, financial, and safety outcomes
Requirements
- Education: Pharm.D. or Bachelor of Science in Pharmacy from an accredited college of pharmacy required
- Certifications: Eligible for pharmacist license through the Board of Pharmacy in both the State of Maine and the State of New Hampshire required
- Experience: Previous pharmacist experience preferred
Qualifications
- Strong interest in drug therapy management for patients with chronic diseases required
- PGY1 Pharmacy residency strongly preferred
