Responsibilities
- Establish and maintain professional relationships with all team members within the medical practice.
- Spearhead patient enrollment, device management, and data interpretation for RPM.
- Collaborate on medication management with supervising providers.
- Participate and lead topic discussions for current staff and residents.
- Precept PGY-1 Students and participate in our Fellowship Program.
- Develop individualized care plans based on specific patient needs.
- Provide recommendations or implement therapy changes, when necessary.
Requirements
- A current, valid pharmacist license to practice in North Carolina.
- Current CPR certification.
- Certification for Immunization.
Qualifications
- Must live in a commutable distance to the posted job location – this role is performed in provider clinics and may require regional travel to a group of clinics.
Skills
- Strong communication and interpersonal skills.
- Ability to work independently and collaboratively.
- Knowledge of chronic care management and remote patient monitoring.
- Experience with medication management and patient care planning.
