Clinical Pharmacist (Per Diem) - Infectious Disease - Atlantic Health Overlook Medical Center
About the role
The Clinical Pharmacist delivers comprehensive, patient-centered pharmaceutical care across a variety of specialty areas, including Behavioral Health, Critical Care, Emergency Medicine, Infectious Disease, Pediatrics/Neonates, Oncology, Inpatient Transitions of Care, Investigational Drug Services, and Opioid Stewardship, and other.
Responsibilities
- Reviewing and optimizing medication therapy, performing medication reconciliation, monitoring therapeutic outcomes, managing adverse drug events, and providing patient and caregiver education.
- Collaborating with interdisciplinary teams to develop individualized care plans, participating in clinical rounds, and providing drug information and pharmacotherapy recommendations.
- Contributing to quality improvement initiatives, policy and protocol development, and clinical projects.
- Conducting medication use evaluations and research, ensuring compliance with institutional, state, and federal regulations to support safe, effective, and evidence-based medication use.
Qualifications
- Graduation from an accredited School of Pharmacy with PharmD required.
- Current active registration and licensure with the State of New Jersey Board of Pharmacy required.
- Previous hospital pharmacy experience preferred.
- PGY1 or PGY2 pharmacy residency training OR 2 years of clinical pharmacy experience in the assigned area.
