Position Summary

Assures the timely and accurate administration of medications and IV solutions to patients pursuant to a therapeutically valid prescribed order and in accordance with all state and federal regulations as well as established peer standards, hospital policies, procedures and protocols.

Essential Responsibilities / Duties

  • Clinically responsible for evaluating therapeutic rationale of all medication orders, providing information/education as needed, and for producing accurate medication administration records.
  • Accurately follows various Clinical Pharmacy Protocols; High Dose Amino glycosides, Weight Based Heparin, Parenteral Nutrition, Adverse Drug Reaction Report, Renal Dosing.
  • Verifies all aspects of medication order & follows AMAR policy Performance Improvement – Medication Process.
  • In response to new information or specific requests, educates medical, nursing, and pharmacy staff in a time and accurate manner.
  • Distributive responsibility for accurate and timely administration of Medications, IV solutions.
  • Makes sure all physician’s orders are processed and medications are delivered efficiently.
  • Monitors hourly distribution rounds and checks all medications.
  • Completes Performance Improvement monitor for Parenteral Nutrition.
  • Loads/unloads medications, troubleshoots common problems, troubleshoots Pyxis/Meditech issues, reviews “meds ordered not loaded” reports for prompt attention.
  • Monitors correct computer entry for charging purposes, reviews entries for appropriate cost/charge.
  • Obtains “short” inventory items or provides therapeutic alternative, provides necessary documentation to assure product availability and appropriate management.
  • Maintains inventory usage, rotation of stock and disposal of outdated, recalled or poorly labeled products.
  • Works collaboratively with Pharmacy Technicians to ensure items are written on list to be ordered from wholesalers.
  • Supervisory/Advisory responsibility to coordinate activities and promote teamwork within the Pharmacy and other hospital departments for optimal patient care.
  • Completes mandatory housewide competencies.
  • Assists in the continuous training of pharmacy staff through observation & mentoring.
  • Provides information/documentation pertaining to performance issues for all professional staff including pharmacy, nursing, physician, other professionals.
  • Mentors new pharmacy staff, both pharmacists, technicians and students.
  • Participates in Performance Improvement activities within the Pharmacy Department and interdisciplinary groups.
  • Participates in assigned special departments projects and committee work (Medical Staff Committees and Hospital Committees/Task Forces).
  • Completes periodic competencies: both housewide and specific clinical competencies.
  • Maintains professional competence through continuing education as required by the Commonwealth of Massachusetts and the Pharmacy Department.
  • Recognizes safety hazards and takes corrective action whenever a hazard exists.
  • Provides for personal safety by having a working knowledge of pharmacy & hospitalwide safety, disaster and infection control plans.
  • Serves as a role model and resource for other staff by sharing knowledge with colleagues.
  • Sets education and learning experiences in areas of identified growth needs and reviews progress during annual evaluation.
  • Responsible for required documentation. Maintains records as required by State and Federal regulations and departmental policies including: controlled substances, dispensing records and billing records as determined by periodic review.
  • Maintains restrictions regarding admittance to pharmacy and observes non-pharmacy personnel allowed admittance.

Job Requirements

  • REQUIRED EDUCATION AND EXPERIENCE: Bachelor's degree in pharmacy or Doctor of Pharmacy (Pharm D) from an Accredited School of Pharmacy Hospital experience including Unit Dose, IV Additives

Is familiar with pertinent pharmacy, nursing service and housewide policies and procedures to facilitate accurate medication administration and optimal therapeutic results Knowledge of Pyxis. Meditech. Knowledge of specific interactions affecting All in One TPN’s Knowledge of Pyxis MedStation and Pharmacy console Is familiar with policy & procedures for reporting loss of controlled substances, including completion of DEA form #106 Compensation Range $50.00- $72.60 This range offers an estimate based on the minimum job qualifications.
