About the role
The role involves collaborating with patient care teams to deliver safe, timely medication therapy at Intermountain Health's Good Samaritan Hospital in Lafayette, Colorado. Pharmacists and pharmacy technicians work together to support patient care through various tasks including unit-dose dispensing, sterile IV compounding, and automated dispensing cabinet support.
Responsibilities
- Reviews patients' medication orders for allergies, drug interactions, and appropriateness of therapy.
- Makes evidence-based interventions to optimize medication use.
- Safely prepares and delivers medications to care areas, ensuring they are stored appropriately.
- Counsels patients on medication use and promotes adherence.
- Documents regulatory requirements clearly and comprehensively.
- Serves as a resource for other healthcare professionals, providing clinical consultations and clarifications.
- Solves problems related to equipment, staffing, drug availability, and patient care.
- Identifies opportunities for improving processes and addressing potential safety hazards.
- Participates in departmental meetings and contributes to initiatives.
- Supports local and enterprise pharmacy leadership initiatives.
Requirements
- Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution.
- Licensed to practice pharmacy in the state of practice and dispense controlled substances.
- Knowledgeable and compliant with state and federal pharmacy laws and regulations.
- Effective communication, people management, and project management skills.
- Previous experience in an integrated healthcare delivery system preferred.
- Board Certification and Immunization certification if applicable.
Qualifications
- Minimum Doctor of Pharmacy or Bachelor of Science in Pharmacy degree.
- Current license to practice pharmacy in the state of practice and dispense controlled substances.
- Knowledge of state and federal regulations and statutes.
- Effective communication, interpersonal, and project management skills.
- Previous experience in an integrated healthcare setting preferred.
- Board Certification and Immunization certification if applicable.
