About the role
The Clinical Pharmacist – Inpatient PRN supports the inpatient pharmacy team at Medina Hospital, providing comprehensive clinical and operational pharmacy services across a community hospital with a strong focus on orthopedic care. Working collaboratively with pharmacists, clinical specialists, physicians, nurses, and other members of the interdisciplinary care team, this role helps ensure safe, effective, and evidence-based medication therapy.
Responsibilities
- Verify and process medication orders
- Perform clinical chart reviews and medication reconciliation
- Enter and clarify medication orders within the electronic health record
- Respond to medication-related questions from caregivers throughout the hospital
- Evaluate medication appropriateness
- Resolve formulary and therapeutic issues
- Partner with the care team to optimize patient outcomes while supporting efficient inpatient pharmacy operations
Requirements
- PharmD or Bachelor’s of Science in Pharmacy from an accredited college of Pharmacy
- Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire
- Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification within 90 days of hire for individuals administering immunizations
Preferred qualifications
- Board certification
- Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience preferred
- Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area
Physical Requirements
- Ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.
- Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
- Position often includes reaching, stretching, stooping, climbing steps and sitting.
- Must have normal or corrected vision.
- Must have ability to clearly communicate verbally by phone or in person.
Pay
Minimum hourly: $67.00
Maximum hourly: $67.00
