100% FTE, evening shifts varying between 12:30 PM – 10:00 PM. 1 in 4 weekends and holiday rotation required. Hours may vary based on the operational needs of the department.
About the Role
We are seeking a Clinical Pharmacist – Critical Care to provide medication therapy management services as part of the interdisciplinary team to patients on inpatient services to assure that drug therapy is safe, effective, efficient, and cost-effective.
Responsibilities
- Actively participate in bedside medical emergencies, providing expertise in medication selection, preparation, procurement, documentation, and medication history review
- Engage in interdisciplinary patient care rounds, including medication reconciliation, therapy monitoring, order verification, patient education, drug information support, and individualized drug dosing
- Provide patient care to a diverse population including critical care, medical and surgical patients
- Teach and precept Pharmacy Residents and PharmD students through structured practical experiences and formal didactic education

Qualifications
- Bachelor's Degree in Pharmacy or a PharmD degree (Required)
- Completion of an ASHP Accredited Residency training program (Preferred)
- 1 year of clinical patient care and orders management experience (Required)
- 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program (Preferred)
Licenses & Certifications
- Licensed to practice pharmacy in the State of Wisconsin within 90 Days (Required)
- Basic Life Support (BLS) within 90 Days (Required)
- Advanced Cardio Life Support (ACLS) within 180 Days (Required)
- Pediatric Advanced Life Support (PALS) within 180 Days (Required)
- Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) (Preferred)
