Clinical Pharmacist II- Per Diem

About the role

Supports the mission of the department by ensuring that the right patients receive the right medication in a safe and timely manner. Clinically reviews medication orders received from prescribers to ensure that the medication, as ordered, is safe to administer to the patient. If not, intervenes with the prescriber on behalf of the patient. Appropriately enters orders into the Pharmacy Information System. Supervises pharmacy technicians and interns to ensure that the daily needs of our patients are met. Provides drug information to medical staff and patients upon request or when need is apparent.

Responsibilities

• Supervises pharmacy technicians and interns to ensure that the daily needs of our patients are being met.
• Educates technicians and interns on safe medication practices.
• Provides drug information to medical staff and patients upon request or when need is apparent.
• Documents requests into the intervention database in accordance with departmental policy.
• Responsible for day-to-day management of drug distribution, including all controlled substances, IV medications, extemporaneously compounded medications, repackaged medications, and investigational drugs.
• Responsible for understanding and utilizing all technology to accomplish the above.
• Serves as a liaison between the pharmacy department and any healthcare provider.
• Responsible for their own professional development, including meeting all requirements for continuing education and keeping abreast of current professional literature from both a clinical and medication safety perspective.
• Actively participates in continuous performance improvement activities of the department, including providing suggestions on how to improve day-to-day operations.
• Performs assigned projects in an efficient and timely manner.
• Demonstrates competency in all practice areas in which pharmacist duties are performed and has completed all required departmental competency requirements.
• Incorporates Lahey Clinic Guiding Principles, Mission Statement, and Goals into daily activities.
• Complies with all Lahey Clinic Policies and behavioral expectations of the department.
• Maintains courteous and effective interactions with colleagues and patients.
• Demonstrates an understanding of the job description, performance expectations, and competency assessment.

• Participates in departmental and/or interdepartmental quality improvement activities.
• Participates in and successfully completes Mandatory Education.
• Performs all other duties as needed or directed to meet the needs of the department.

Requirements

• Graduate of an accredited college of pharmacy with a current MA registration in Pharmacy.
• Valid MA pharmacist license.
• One year of hospital pharmacy experience is required.
• Working knowledge of all basic aspects of health system pharmacy (including drug therapeutics and sterile compounding).

Pay

The pay range for this position is $59.74 - $80.40 per hour. Actual compensation is determined based on several factors, including seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
