Clinical Pharmacist Per Diem

Responsible for supervising the filling and dispensing of medication orders including the proper handling, usage, and storage of drugs. Provides clinical services and drug information to physicians, residents, and nurses ensuring appropriate drug usage. Participates in the training of pharmacy technicians and co-op students. Engages in quality improvement projects related to area of pharmacy expertise.

Responsibilities

• Participates in daily patient rounds to provide input on the selection and rational approach to pharmacotherapy, including pharmacologic and pharmacokinetic principles, drug toxicities and side effects, and individual dosage recommendations.
• Evaluates all drug therapy for appropriate indications, dosage, drug interactions, and allergies; monitors patients’ regimen for effectiveness and adverse drug events, intervening as appropriate.
• Participates on inter- and intradepartmental committees to integrate and coordinate pharmacy, nursing, and provider staff activities.
• Participates in the orientation and training of all new personnel.
• Interviews patients to obtain accurate and comprehensive medical history, including prescribed medications, over-the-counter medications, adverse drug reactions, and allergies.
• Prepares drug monographs and reviews drugs requested for admission to the Hospital’s Formulary.
• Assists in reviewing drug costs and making necessary adjustments as required.
• Conducts in-service education lectures for medical staff, nurses, pharmacists, students, and other healthcare personnel regarding medication management, procedures, and policies.
• Assists in formulating and updating departmental policies and procedures, as well as pharmacy-nursing policies and procedures.
• Occasionally assists with cleaning pharmacy delivery carts, medication bins, shelves, patient medication cassettes and drawers, and other pharmacy equipment.
• Performs related duties as needed.

Requirements

• Doctor of Pharmacy from an accredited college of pharmacy with 2-5 years of hospital pharmacy experience.
• Current registration as a Pharmacist in Massachusetts required by the hire date.

Preferred Qualifications

• PGY1 residency preferred.

Skills

• Communication skills for concise dissemination of information to patients, nurses, and physicians.
• Administrative skills to coordinate personnel and pharmacy operations.
• Teaching skills for preceptorship of pharmacy students.

Pay

Compensation range: $52.40–$75.96 per hour. This range is based on minimum job qualifications and considers factors such as education, experience, skills, certifications, internal equity, and market competitiveness. Geographic location may modify the range.
