About the Role

This role initiates and ensures the quality of pharmacy and drug distribution services in all patient care areas, assuming direct responsibility for medication distribution, IV admixtures, and the safety and efficacy of prescribed drug therapy in accordance with laws and regulations governing pharmacists and the practice of pharmacy.

Responsibilities

  • Practice in accordance with state and federal laws, rules, and regulations, including but not limited to those set by the Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and Drug Administration, and the Drug Enforcement Administration.
  • Maintain certification and credentials in accordance with the Board of Registration and adhere to institutional policies and procedures.
  • Review and verify medication orders considering patient-specific parameters such as diagnosis, weight/body surface area, age, allergies, organ function, laboratory values, medication duplication, and drug interactions.
  • Clarify orders deemed inappropriate, incomplete, or unclear with the prescriber in a timely manner.
  • Document and communicate pertinent information and discussions with the healthcare team and pharmacy staff to ensure continuity of care.
  • Provide drug information to physicians, nurses, and other healthcare professionals.
  • Check and verify sterile and non-sterile medications.
  • Supervise the activities of students, technicians, and residents within the pharmacist area of practice.
  • Promote a culture of patient safety by submitting medication-related events and near misses.
  • Complete daily pharmacy surveillance tool monitoring, including follow-up and documentation, as well as other daily patient care initiatives and relevant reports.
  • Work collaboratively with all pharmacists, pharmacy technicians, and pharmacy personnel.
  • Uphold a culture of respect and teamwork across the Pharmacy Department.
  • Participate in the onboarding and training of new hires and precept pharmacy students and/or residents through core clinical rotations or learning experiences.
  • Regularly participate in department initiatives, including supporting formulary stewardship, presenting new medication requests at Pharmacy & Therapeutics Committee, creating and maintaining policies and procedures, conducting workflow assessments, and developing departmental tools and resources.
  • Support departmental workgroups to improve process and quality outcomes.
  • Support the development and revision of departmental clinical guidelines.
  • Perform other duties as assigned.

Requirements

  • PGY-1 Residency OR 1-3 years of relevant hospital experience.

  • Age-Based Competencies: Competence in providing patient care to the following age groups: Neonatal (birth to 6 months), Youth (6 months to 16 years), Young adult (16-30 years), Middle Age (30-60 years), Elderly (60+).

Physical Nature of the Job

Sedentary work: Exerting up to 10 pounds of force occasionally in carrying, lifting, pushing, pulling objects. Sitting most of the time, with walking and standing required only occasionally.

Pay

The pay range for this position is $61.29 - $82.48 per hour. The actual compensation is determined based on several factors, including seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices.
