Clinical Pharmacist - Hematology Oncology

Role Overview: The Pharmacist delivers safe, effective, and cost-efficient medication therapy, developing and monitoring evidence-based pharmacotherapy care plans to improve patient outcomes and quality of life. Key Responsibilities Provide pharmaceutical care in designated patient care areas, coordinating resources for efficient service.Interpret and clarify physicians' medication orders, review patient medication profiles for safety and interactions.Assess medication orders for formulary, therapeutic appropriateness, and suggest alternatives.Collaborate with medical staff on drug therapy issues, monitor laboratory results, and advise on treatment plans.Provide drug information, patient discharge counseling, and participate in rounds with medical teams.Calculate dosages, verify orders, and prepare sterile and compounded medications.Report supply needs, supervise pharmacy staff and students, and ensure compliance with departmental standards.Coordinate with healthcare professionals to resolve pharmacy-related issues and update policies as needed. Qualifications & Skills Bachelor's Degree in Pharmacy required; Doctoral Degree preferred.Strong knowledge of pharmacotherapy, medication safety, and lab monitoring.Excellent communication, collaboration, and supervisory skills.Ability to interpret complex medical information and perform precise calculations.Experience in sterile compounding and medication management.
