Clinical Pharmacist - Central Distribution

Clinical pharmacists on the central distribution team are responsible for a variety of clinical and distributive responsibilities, including staffing central pharmacies and satellite locations, overseeing pharmacy technicians, and ensuring safe and effective medication use across all patient care areas.

Schedule

This position works 8-hour shifts, averaging 3 evenings per month (start time between 1400-1500) and rotating approximately every 4th weekend.

Responsibilities

• Actively participates in organizational and departmental performance improvement efforts to strengthen systems, processes, policies, and procedures, introducing new ideas to improve pharmaceutical care in clinical areas and the pharmacy department.
• Actively participates on the multidisciplinary team by attending patient care rounds and providing patient-focused pharmaceutical care, assessing and monitoring drug therapy to ensure safe and cost-conscious treatment regimens.
• Supports department staffing needs by flexing to other areas or adjusting schedules to assist in coverage of supplemental shifts when the primary team is in need.
• Ensures timely medication order processing by participating in order entry, verification, and reviewing medication orders for resolution of issues.
• Completes patient-specific medication profile reviews, executing protocol-based drug therapy (e.g., pharmacokinetic dosing, IV to PO conversions) with appropriate documentation and adjusting doses for patient-specific parameters such as renal or hepatic dysfunction.
• Reports medication incidents and adverse drug reactions in Patient Safety Net (PSN) and participates in reviews to establish safe medication systems.
• Participates in medication reconciliation at admission, transfer, and/or discharge and provides patient education when appropriate.
• Shares pharmaceutical expertise by coordinating and providing educational presentations or in-services for pharmacy and non-pharmacy staff.
• Supports pharmacy residency programs, clerkship students, and interns by precepting rotations, providing feedback, participating in training, offering mentorship, and collaborating on presentations and projects.
• Performs the professional, clinical, and technical competencies of the assigned unit or department.

Skills and duties may vary depending on the department or unit. Other duties may be assigned as required.

Requirements

• Bachelor’s Degree in Pharmacy from an accredited college or university.
• Licensed Pharmacist – State Board of Pharmacy in the state of primary practice site.
• Basic Cardiac Life Support (BLS or BCLS) – American Heart Association (AHA).
• Advanced Cardiac Life Support (ACLS) – American Heart Association (AHA) certification as necessary.
• Pediatric Advanced Life Support (PALS) and/or Neonatal Resuscitation Program (NRP) certification as necessary.

Preferred Qualifications

• Completion of an accredited pharmacy postgraduate training program (PGY-1 and/or PGY-2 residency program).
• Board of Pharmaceutical Specialties certification in a chosen area of specialty.
