About the role

This role determines patient suitability for home care, designs and monitors patient-specific pharmacy care plans, and ensures continuity of pharmaceutical care. You will prepare and dispense medications, manage medication administration equipment, and provide drug information to healthcare providers and patients. The position also involves utilizing pharmacy support personnel effectively and maintaining confidentiality of patient and proprietary information.

Responsibilities

• Determine the suitability of individual patients for home care by collecting and organizing patient-specific information and assessing patients in accordance with Option Care policies.
• Design, recommend, monitor, and evaluate patient-specific pharmacy care plans for home care patients, including making modifications to prescribed medication therapy or monitoring parameters.
• Collect and organize patient-specific information to prevent, detect, and resolve medication-related problems, and identify appropriate monitoring parameters.
• Complete nutritional assessments and make appropriate recommendations.
• Modify pharmacy care plans based on evaluation of monitoring data and other pertinent patient-specific information, demonstrating competence in pharmacokinetic principles and assessing patient laboratory values.
• Provide medication-use education to Option Care staff, other professionals, patients, and/or their caregivers using effective patient education techniques.
• Ensure continuity of pharmaceutical care to and from the home and other patient-care settings by systematically communicating pertinent patient information.
• Prepare and dispense medications using appropriate techniques, adhering to safety and quality assurance practices, and complying with state/federal laws and USP standards, including DEA controlled substance regulations.
• Manage the use, maintenance, and troubleshooting of medication administration equipment and medication-related equipment, including programming, maintaining, and advising others on its use.
• Participate in assessing, managing, and reporting medication-related incidents and equipment errors/failures, and complete QAR documents according to Option Care policy.
• Monitor and report discrepancies in perpetual inventory systems for schedule II narcotics and other medications subject to diversion.
• Provide concise, applicable, and timely responses to drug information requests from healthcare providers and home care patients, formulating responses based on literature analysis.
• Attend educational seminars or participate in continuing education opportunities to promote professional competency and comply with state-mandated requirements.
• Manage the use of investigational drug products according to established regulations and Option Care policies.
• Utilize pharmacy support personnel effectively by prioritizing workload, organizing workflow, and ensuring the accuracy of their work while complying with accreditation, legal, regulatory, and safety requirements.
• Maintain confidentiality of patient and proprietary information, observing legal and ethical guidelines for safeguarding information.

Requirements

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following:
  • At least one year of experience in sterile IV admixture procedures.
  • Knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM).
  • Completion of an ASHP accredited Pharmacy Residency Program.
  • Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
  • Equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services.
• Experience providing customer service to internal and external customers, including meeting quality standards and evaluating customer satisfaction.
• Basic level skills in Microsoft Word, Excel, and PowerPoint.
• Willing to travel up to 5% of the time for business purposes (within state).

Preferred Qualifications

• Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
• Experience in home care, hospital, or other clean room settings.
• At least 1 year of experience in indirect management of team members, including assisting in development, training, and assignment of work/projects.

Pay

Salary to be determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Pay range is $102,221.94 - $170,369.91.
