About the role
The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with company policies, professional standards, and regulatory requirements. This entry-level position coordinates patient services with the Nursing Department, monitors patient progress, and educates patients, caregivers, and professionals on medication use.
Responsibilities
- Determine the suitability of individual patients for home care by collecting and organizing patient-specific information and assessing their medical condition and prescribed therapy.
- Design, recommend, monitor, and evaluate patient-specific pharmacy care plans, including modifications to prescribed medication therapy or monitoring parameters.
- Complete nutritional assessments and make appropriate recommendations.
- Modify pharmacy care plans based on monitoring data and patient-specific information, demonstrating competence in pharmacokinetic principles and laboratory value assessment.
- Provide medication-use education to staff, professionals, patients, and caregivers using effective techniques.
- Ensure continuity of pharmaceutical care between the home and other patient-care settings through systematic communication of pertinent information.
- Prepare and dispense medications using appropriate techniques, adhering to safety, quality assurance, and regulatory practices, including DEA controlled substance regulations and USP standards.
- Manage the use, maintenance, and troubleshooting of medication administration and related equipment, including reporting medication-related incidents and equipment errors.
- Provide timely and applicable responses to drug information requests from healthcare providers and patients, based on literature analysis.
- Manage investigational drug products according to established regulations and company policies.
- Utilize pharmacy support personnel effectively by prioritizing workload and ensuring accuracy in their work.
- Maintain confidentiality of patient and proprietary information in compliance with legal and ethical guidelines.
Requirements
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license as granted by the State Board of Pharmacy.
- One of the following:
- At least one year of experience in sterile IV admixture procedures.
- Knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM).
- Completion of an ASHP accredited Pharmacy Residency Program.
- Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
- Equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services.
- Experience providing customer service to internal and external customers.
- Basic proficiency in Microsoft Word, Excel, and PowerPoint.
- Willingness to travel up to 5% of the time for business purposes (within state).
Preferred Qualifications
- Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
- Experience in home care, hospital, or other clean room settings.
- At least 1 year of indirect management experience, including assisting in the development, training, and assignment of work/projects to team members.
Pay
Salary is determined by the applicant’s education, experience, knowledge, skills, and abilities, as well as internal equity and market alignment. Pay range: $102,221.94 - $170,369.91.
