Clinical Pharmacist

This role will support clinical priorities focused on quality, patient-centered outcomes, and effective care delivery. You will collaborate with clinical leaders, operational teams, and cross-functional stakeholders to improve standards, workflows, and execution. The position offers the opportunity to strengthen care quality, team performance, and measurable outcomes within a healthcare environment.

Responsibilities

• Analyze medication utilization data for polypharmacy trends
• Develop strategies to optimize therapies and improve adherence
• Collaborate with care teams on interventions for cost-effectiveness
• Monitor and report impacts of medication management strategies
• Conduct medication reviews and address therapy issues effectively
• Participate in population health initiatives related to medication use
• Educate providers and patients on medication best practices

Qualifications

• PharmD from ACPE-accredited College of Pharmacy required
• Completion of ASHP-accredited PGY-1 Pharmacy Residency; PGY-2 preferred
• Active pharmacy license without restrictions
• Minimum 3 years of clinical pharmacy experience in ambulatory care, ideally with value-based care
• Proficiency in clinical pharmacy practice and medication therapy management
• Skilled in medication data analysis and quality improvement using analytics tools

Pay

$115,000 – $125,000 annually
