About the role
This role will support clinical priorities focused on quality, patient-centered outcomes, and effective care delivery. You will collaborate with clinical leaders, operational teams, and cross-functional stakeholders to improve standards, workflows, and execution. Responsibilities
- Analyze medication utilization data for polypharmacy trends
- Develop strategies to optimize therapies and improve adherence
- Collaborate with care teams on interventions for cost-effectiveness
- Monitor and report impacts of medication management strategies
- Conduct medication reviews and address therapy issues effectively
- Participate in population health initiatives related to medication use
- Educate providers and patients on medication best practices
Qualifications
- PharmD from ACPE-accredited College of Pharmacy required
- Completion of ASHP-accredited PGY-1 Pharmacy Residency; PGY-2 preferred
- Active pharmacy license without restrictions
- Minimum 3 years of clinical pharmacy experience in ambulatory care, ideally with value-based care
- Proficiency in clinical pharmacy practice and medication therapy management
- Skilled in medication data analysis and quality improvement using analytics tools
Pay
$115,000 – $125,000 annually
