Clinical Pharmacist

About the role

The Clinical Pharmacist will work at the Toledo - OH Community Based Outpatient Clinic (CBOC) within the VA Ann Arbor Healthcare System (VAAAHS).

Responsibilities

• Handles routine medication-related activities in accordance with local, VISN, and national policies and regulations.
• Reviews, interprets, and verifies medication orders for appropriateness.
• Processes and fills medication orders.
• Interacts with and makes recommendations to other clinical staff regarding medication therapy.
• Makes recommendations and interventions based on patient's medications, allergies, labs, and other pertinent information from the medical record.
• Documents recommendations and interventions.
• Takes health and medication histories.
• Performs medication reconciliation.
• Provides drug information.
• Affords formulary management including therapeutic substitutions, non-formulary reviews, and medication usage evaluations.
• Assists in medical emergencies.
• Documents and assesses adverse drug events (ADEs).
• Assists in medical emergencies including code teams and emergency preparedness activities.

Requirements

• Graduate of an ACPE-accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
• Licenses: Full - current and unrestricted license to practice pharmacy in a State - Territory - Commonwealth of the United States (i.e. - Puerto Rico) - or the District of Columbia.
• Knowledge of professional pharmacy practice, laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
• Skills in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
• Ability to communicate orally and in writing to both patients and health care staff.

Qualifications

• Experience: 11 years of experience equivalent to the next lower grade level, or 2 Completion of an ACPE-accredited Pharm.D. program AND Demonstrated Knowledge - Skills - and Abilities.
• Knowledge: Knowledge of professional pharmacy practice.
• Skills: Ability to communicate orally and in writing to both patients and health care staff; Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security; Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
