Responsibilities
- Ensure medications, supplies, and drug information are delivered accurately, promptly, and in full compliance with Joint Commission requirements and other regulatory standards.
- Review and evaluate drug regimens—including complete medication profiles—to identify issues that may require pharmacy intervention or ongoing monitoring.
- Collaborate with multidisciplinary teams to guide the optimal use of medications and to support coordinated care plans for individual patients and patient groups.
- Provide education to patients and caregivers, build positive relationships with internal and external partners, collaborate effectively with pharmaceutical service representatives, and contribute to quality improvement initiatives across the organization.
Qualifications
- Earned at least a Bachelor’s Degree from an accredited pharmacy program
- Indiana Pharmacist License
- CPR/BLS
- Two (2) years of hospital pharmacy experience, preferred
