Responsibilities

• Ensure medications, supplies, and drug information are delivered accurately, promptly, and in full compliance with Joint Commission requirements and other regulatory standards.
• Review and evaluate drug regimens—including complete medication profiles—to identify issues that may require pharmacy intervention or ongoing monitoring.
• Collaborate with multidisciplinary teams to guide the optimal use of medications and to support coordinated care plans for individual patients and patient groups.
• Provide education to patients and caregivers, build positive relationships with internal and external partners, collaborate effectively with pharmaceutical service representatives, and contribute to quality improvement initiatives across the organization.

Qualifications

• Earned at least a Bachelor’s Degree from an accredited pharmacy program
• Indiana Pharmacist License
• CPR/BLS
• Two (2) years of hospital pharmacy experience, preferred
