Responsibilities

• Engaging with patients through outbound calls to provide personalized medication support, with an expectation of approximately 100 calls per day
• Performing comprehensive medication reviews
• Delivering medication therapy management services
• Accurately and efficiently transcribing patient-specific prescription and diagnosis information, along with recommendations, into a web-based application to generate personalized patient documents
• Participating in continuous learning and training sessions to enhance skills and stay current with best practices

Requirements

• A current and valid Pharmacist License in good standing
• At least 4 years of experience as a clinical professional specializing in medication therapy management
• Familiarity with key quality measures related to medication use and patient safety
• A strong understanding of pharmaceutical and medical vocabulary
• Demonstrated clinical experience in medication review processes
• Fluency in spoken and written English
• Excellent verbal and written communication skills
• Proficiency with technology and various software applications
• Self-directed and highly organized
• Ability to maintain strict confidentiality and discretion
• Exceptional attention to detail
• A proactive approach and a strong sense of urgency

Nice-to-Have Qualifications

• Management experience
• Experience with customer relationship management (CRM) software
