Responsibilities
- Engaging with patients through outbound calls to provide personalized medication support, with an expectation of approximately 100 calls per day
- Performing comprehensive medication reviews
- Delivering medication therapy management services
- Accurately and efficiently transcribing patient-specific prescription and diagnosis information, along with recommendations, into a web-based application to generate personalized patient documents
- Participating in continuous learning and training sessions to enhance skills and stay current with best practices
Requirements
- A current and valid Pharmacist License in good standing
- At least 4 years of experience as a clinical professional specializing in medication therapy management
- Familiarity with key quality measures related to medication use and patient safety
- A strong understanding of pharmaceutical and medical vocabulary
- Demonstrated clinical experience in medication review processes
- Fluency in spoken and written English
- Excellent verbal and written communication skills
- Proficiency with technology and various software applications
- Self-directed and highly organized
- Ability to maintain strict confidentiality and discretion
- Exceptional attention to detail
- A proactive approach and a strong sense of urgency
Nice-to-Have Qualifications
- Management experience
- Experience with customer relationship management (CRM) software
