Clinical Pharmacist - Adult Medicine

Responsibilities

• Reviews patient information to evaluate appropriateness of drug therapy and dosing regimens as well as absence of contraindications.
• Selects drug products that are effective, cost beneficial, and promote patient compliance.
• Suggests non-drug therapy when appropriate.
• Aids the medical staff in selecting therapy regimens, which are cost effective and meet individual patient needs.
• Suggests appropriate alternatives for non-formulary drugs.
• Recommends drug discontinuation or dosage alteration when indicated.
• Identifies potentially significant drug-drug, drug-food, drug-laboratory, and drug disease interactions and communicates clinically relevant interactions to prescribers.
• Affirms compliance and follow-up with adverse reaction reporting and medication occurrence reporting programs.
• Liaison/Committee Participation
• Works with medical staff in developing protocols and dosing guidelines.
• Serves on one or more Medical Staff Committees or Subcommittees.
• Actively contributes to accomplishing the goals of the Committee or Subcommittee.

Qualifications

• Doctor’s Degree in Pharmacy from an accredited School of pharmacy and continuing education requirements of the Mississippi State Board of Pharmacy.
• Completion of clinical residency or equivalent experience or training.
• Minimum of three years' clinical expertise required.
• Demonstrates ability to recognize patients' individual needs based on medical conditions, age (infants, pediatrics, adolescents, young adults, middle-aged and geriatric), limitations and planned procedures.
• Requires oral and written communication skills, leadership skills and professional affiliations.
• Licensed as a pharmacist in Mississippi, and controlled substance license.
• Board Certification in Pharmacotherapy (or subspeciality) is desired.
