About the role

An Outcomes Manager in this role will coordinate data collection, ensure documentation compliance, and manage IRF-PAI submissions within CMS guidelines.

Responsibilities

• Coordinate PPD data collection with the interdisciplinary team.
• Ensure supportive documentation for QIs is complete in the medical record.
• Complete admission and discharge IRF-PAIs within CMS-designated timeframes.
• Abstract required information from medical records and summarize on the IRF-PAI.
• Enter data into the CMS-accepted software/program within designated timeframes.
• Maintain and update the IRF-PAI log; verify accuracy against the patient location roster.

Requirements

• Clinical degree with current license to practice as an OT, PT, RN, SLP, or MSW.
• QI competency required.
• Working knowledge of state and federal guidelines; familiarity with healthcare workflows and documentation.
• Basic computer and data entry proficiency.
• Strong clinical assessment/analysis, effective verbal and written communication, excellent interpersonal skills, and ability to collaborate across disciplines.
