Responsibilities

• Collects, tracks, trends and monitors data
• Reviews outcomes, compliance and identifies opportunities for improvement
• Presents findings and makes recommendations to interdisciplinary health care team
• Serves as clinical case manager
• Conducts new admission assessments as census dictates
• Completes a clinical record review for current and new admissions

Requirements

• Current CA license as registered nurse
• BSN preferred
• Minimum of three years supervisory experience
• Experience leading within a Skilled Nursing facility
• Demonstrated competence with use of electronic medical record, PCC preferred
• Ability to flex schedule based on facility needs
