Responsibilities

• Guide patients through the healthcare journey by providing patient-centered education, emotional support, and clinical guidance.
• Ensure patients understand their care options and feel supported at each step.
• Coordinate multidisciplinary appointments, tests, and treatments.
• Serve as a key liaison between patients, healthcare providers, and external resources.
• Perform thorough clinical assessments and precise documentation.
• Engage in proactive communication to streamline patient care.
• Connect patients to relevant resources to ensure comprehensive and compassionate care through the treatment process.

Requirements

• Associate's degree from an accredited School of Nursing program.
• State of New Mexico Nursing License or valid multi-state compact license.
• BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care.
• 3-5 years of clinical nursing experience.

Preferred Qualifications

• Interdisciplinary teamwork experience.
• Bachelor's of Nursing or Healthcare Management.
