About the role
This position provides comprehensive clinical and administrative support to healthcare providers, including direct patient care, electronic health record management, coordination of referrals and prior authorizations, and administrative functions essential to clinic operations.
Responsibilities
- Initiates the clinical program project plans as outlined and establishes and maintains project completion schedules.
- Serves as a consistent contact for patient/family throughout the process of diagnosis and treatment.
- Provides individualized information, support and education.
- Coordinates appointments for consultations and physician visits.
- Notifies primary care and specialty care providers when patient is admitted to the hospital if applicable.
- Reinforces and assesses pre- and post-operative instructions and provides further education as appropriate.
- Guides and coordinates care for patients and their caregivers through the continuum of care.
- Ensures data is collected, reviewed and analyzed monthly for display on dashboards.
- Develops and implements action plans accordingly based on outcomes data.
- Collaborates with other disciplines using an integrated approach toward achieving optimal patient outcomes.
- Responsible for PI and other metrics to determine effectiveness with all levels of care providers to optimize the patient care experience.
- Compiles and analyzes quantitative and qualitative data, including benchmark data, and shares analysis with the leadership team and senior management.
- Identifies and anticipates patient and/or provider needs and develops evidence-based quality programs to meet those needs.
- Ensures delivery of quality services for patients.
- Interacts with clinic leadership and Marketing to set up activities and education at the request of the hospital and the request of community groups and organizations.
- Assists in the development and presentation of educational programs geared toward primary care physicians, surgeons, staff and community.
- Maintains a thorough knowledge and understanding of evidence based/best practice.
- Revises and monitors educational materials to ensure that they are coordinated, understandable and meet the program standards.
- Protects patient confidentiality by promoting professional staff communications.
- Supports the hospital mission, vision, values, policies, and procedures.
- Conducts comprehensive nursing assessments and specialty-specific nursing visits.
- Performs post-operative wound checks, dressing changes, and wound care management as applicable.
- Administers medications, injections, immunizations, and treatments as ordered by physicians.
- Aids in procedures and examinations specific to the specialty area.
- Maintains vital signs and documents patient responses to treatments.
- Recognizes and responds appropriately to changes in patient condition.
- Performs diagnostic tests including EKGs, spirometry, glucose monitoring, and CLIA-waived laboratory tests as appropriate to scope of practice.
- Conducts focused nursing visits for blood pressure monitoring, suture/staple removal, injection appointments, weight checks, and routine procedures.
- Performs other related duties as assigned.
- Monitors electronic health record (EHR) messages in accordance with clinic protocols.
- Provides telephone triage for patient calls regarding urgent/emergent care and clinical questions.
- Documents all patient interactions, assessments, and interventions accurately in EHR.
- Manages referrals and coordinates care between specialties.
- Responds to patient portal messages and telephone calls within established timeframes.
- Processes prior authorization requests and coordinates with referral staff as needed.
- Processes prescription refill requests and coordinates with providers as needed.
- Provides comprehensive patient and family education on disease processes, medications, and treatment plans.
- Develops and delivers specialty-specific education materials and instructions.
- Counsels patients on post-operative care, medication compliance, and lifestyle modifications specific to the specialty.
- Facilitates patient understanding of treatment options and discharge instructions.
- Supports team collaboration and clinical support.
- Supports the hospital mission, vision, values, policies, and procedures.
- Works closely with physicians, nurse practitioners, and physician assistants to ensure coordinated care.
- Participates in multidisciplinary team meetings and care planning conferences.
- Mentors and provides guidance to medical assistants and other support staff.
- Communicates effectively with ancillary departments and external healthcare providers.
- Collaborates effectively with medical assistants, rooming patients when clinic flow requires additional support.
- Affords assistance with examination room preparation and ensures adequate inventory of medical supplies.
- Supports front office operations including appointment scheduling and phone coverage as needed.
- Adheres to all clinic policies, procedures, and safety protocols.
- Maintains compliance with regulatory requirements including OSHA, HIPAA, and DNV standards.
- Participates in quality improvement initiatives and clinical outcome measurements.
- Ensures proper infection control practices and sterilization procedures as indicated.
- Maintains a safe and therapeutic environment for patients and staff.
- Participates in required education for DNV programs as applicable to position (reference program education curriculum).
- Performs other related duties as assigned.
Qualifications
- Education: Graduate of an accredited school of professional nursing.
- Preferred: BSN.
- Licenses/Certifications: Current, unrestricted Oregon Registered Nurse license. Current BLS certification. Preferred: ACLS (Advanced Cardiac Life Support) certification, Specialty certification relevant to assigned clinic area (e.g., CPN for Pediatrics, CGRN for Gastroenterology).
- Experience: The ability to work with a culturally diverse population. Preferred: Bilingual and/or English Spanish speaking. Minimum 2 years of clinical nursing experience. Experience with Epic electronic health record system.
- Knowledge: Knowledge of computers, phones, and other office equipment. Knowledge in clinical equipment including but not limited to: EKG, vital sign monitoring devices, glucometer, scales, various lab testing equipment, etc.
- Skills: Strong assessment and critical thinking skills. Excellent verbal and written communication abilities. Proficiency in patient education and health promotion. Ability to work independently and prioritize multiple tasks effectively.

Tactile perception is also important for assessing physical characteristics such as temperature, size, shape, or texture during patient care. Motor coordination is required for reaching, grasping, handling, and performing fine motor tasks necessary for the use of medical equipment and procedures. The position also involves occasional stooping, kneeling, crouching, or crawling when assisting patients or accessing equipment. Walking and standing are typically continuous throughout the shift. The role requires the ability to lift to 50 pounds independently and involves frequent pushing, pulling, carrying, or repositioning of objects or patients weighing up to 25 pounds. There may also be occasional lifting or transferring of individuals weighing over 100 pounds, with the expectation that appropriate lifting techniques, mechanical aids, or team assistance will be used to ensure safety. - Working Conditions: This position is performed primarily in an indoor healthcare environment, such as a hospital, clinic, or long-term care facility. While the work setting is protected from outdoor weather conditions, it may be subject to variable indoor temperatures. The role involves frequent exposure to infectious and contagious diseases, hazardous materials, and medical equipment, with the expectation that all staff follow appropriate safety protocols and utilize personal protective equipment (PPE) as required. The health care environment can be fast-paced and unpredictable, requiring the ability to respond quickly to changing circumstances, including high-pressure or emergency situations. The work setting also involves multiple simultaneous demands and varying levels of noise due to medical equipment, staff activity, and patient needs.
