Responsibilities
As a Clinical Navigator, you will:
- Meet people at their worst of times and share our success and potential success with them.
- Facilitate the referral conversion by working collaboratively with various teams including the Admissions Manager, admission staff, nursing, and other internal and external staff.
- Provide feedback and recommendations for program development and quality improvement initiatives related to customer service, the referral and admission process, and patient and referral source satisfaction.
- Develop and maintain excellent relationships with all stakeholders including prospective patients, family members, physicians, ICU and floor nurses, discharge planners, case managers, and payer representatives.
Requirements
Qualifications:
- Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
- Minimum of two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.
