Clinical Navigator - PRN Weekends | Golden Rehab
About the Role
As a Clinical Navigator, you'll be a trusted resource and the face of our hospital, helping patients and families understand their options and the potential for recovery during their rehabilitation journey. You'll meet patients during some of the most challenging moments of their lives and have the opportunity to provide guidance, hope, and support while helping connect them with the specialized care they need.
Responsibilities
- Build and maintain strong referral relationships within an assigned territory.
- Serve as a clinical resource and representative of PAM Health to patients, families, physicians, case managers, discharge planners, nurses, and other referral partners.
- Complete clinical assessments of prospective patients to determine appropriateness for admission.
- Cultivate new referral opportunities and maintain existing referral relationships.
- Analyze referral and admission trends to identify opportunities for growth.
- Collaborate closely with the Business Development team, including the Admissions Manager and admissions staff, to facilitate referral conversion and admissions.
- Partner with nursing, therapy, physicians, and other internal teams to support a seamless transition of care.
- Educate patients, families, and referral sources about PAM Health's programs, services, and clinical capabilities.
- Develop and maintain strong relationships with healthcare stakeholders, including physicians, ICU and floor nurses, case managers, discharge planners, payer representatives, patients, and families.
- Provide feedback and recommendations to support program development, quality improvement, and exceptional customer service.
- Monitor patient and referral source satisfaction and identify opportunities to improve the referral and admission experience.
- Represent PAM Health professionally throughout the community and assigned territory.
Requirements
- Minimum of two years of experience in your licensed clinical discipline.
- Rehabilitation, LTACH, acute care, or healthcare marketing/business development experience preferred.
- Strong communication, relationship-building, organization, and presentation skills.
- Ability to work collaboratively with clinical and business development teams.
- Results-oriented mindset with a passion for helping patients and families navigate their healthcare journey.
Qualifications
- Degree in a health-related field from an accredited college or university preferred.
- Current professional license in one of the following fields required:
- Registered Nursing (RN)
- Respiratory Therapy (RT)
- Physical Therapy (PT)
- Occupational Therapy (OT)
- Speech-Language Pathology (SLP)
