About the role
Manages patient discharges, develops home care plans, and acts as a liaison with hospitals, nursing homes, and outpatient services. Coordinates medical equipment and supplies delivery, supports recruitment, staff training, scheduling, and performance management. Supervises clinical personnel, ensuring quality care and compliance with standards.
Responsibilities
- Participates in discharge planning discussions with patients, families, and care teams.
- Reviews patient needs through daily rounds and monitors discharge processes.
- Compiles clinical data, reports patient assessments, and assists with staff orientation and education.
- Evaluates referrals, develops marketing strategies, and manages medical supply orders.
- Supports recruitment, trains staff, addresses performance issues, and evaluates personnel.
- Performs administrative duties, including data collection and participation in hospital committees.
Requirements
- Bachelor’s degree in nursing or equivalent; Master’s preferred.
- Minimum five years’ home care nursing experience; leadership and hospital case management experience preferred.
- Valid RN or PT license; CPR/BLS certification preferred.
- Valid driver’s license and age of at least 21.
