Clinical Manager (RN) - Full Time

About the Role

Managing patient discharges, developing home care plans, and coordinating post-hospital needs, this role serves as a key liaison between patients, families, healthcare teams, and community agencies.

Responsibilities

• Participate in discharge planning discussions and develop continuity of care plans.
• Conduct daily rounds, review discharge needs, and monitor patient progress.
• Compile monthly statistics, maintain medical records, and report assessments to medical staff.
• Assist with patient referrals, evaluate home care needs, and coordinate medical equipment delivery.
• Support recruitment, staff training, and performance evaluations of team personnel.
• Provide education, address staff concerns, and participate in hospital committees.

Requirements

• Bachelor's Degree in nursing or related field; Master’s preferred.
• Minimum five years' home care nursing experience; leadership experience preferred.
• Valid RN or PT licensure; CPR/BLS certification preferred.
• Strong communication, organizational, and supervisory skills.
• Ability to coordinate multidisciplinary teams and manage multiple responsibilities effectively.
