About the Role
Responsible for managing patient discharges, developing home care plans, and coordinating post-hospital needs. Acts as a liaison with hospitals, nursing homes, outpatient services, and physician offices. Arranges medical equipment and supplies, supports recruitment, training, staff scheduling, and performance management. Supervises personnel activities, ensuring quality care and compliance with standards.
Responsibilities
- Participate in discharge planning discussions and coordinate care with healthcare teams
- Conduct daily rounds, review discharge needs, and monitor patient progress
- Compile statistical reports, maintain medical documentation, and ensure compliance
- Assess referrals, assist in marketing strategies, and evaluate equipment needs
- Lead staff recruitment, training, evaluations, and address personnel issues
- Perform additional duties as assigned to support department operations
Requirements
- Bachelor's Degree in nursing or related field; Master's preferred
- Registered Nurse (RN) or Physical Therapist (PT) licensure
- Minimum of five years home care nursing experience
- Leadership and hospital case management experience preferred
- Valid driver's license, at least 21 years old
