Core Responsibilities
- Advocates for and helps clients gain access to resources while confronting their personal issues.
- Counsels and provides crisis intervention for patients, including assessment and treatment of behavioral problems.
- Coaches patients on how to manage ongoing health conditions.
- Provides therapy to help patients make positive behavior changes.
- Adheres to University and unit-level policies and procedures and safeguards University assets.
Department Specific Functions
- Responsible for social work services at the multidisciplinary ALS and MDA multispecialty and regular clinics.
- Conducts suitability assessments of patients in the multidisciplinary clinic.
- Provides psycho-social assessment and diagnosis of behavioral disabilities with recommendations.
- Provides information and referral to public or private agencies with appropriate follow-up.
- Provides information to patients about appropriate insurances and disability process.
- Completes disability forms, workers compensation, Medicaid waiver forms and other governmental program forms.
- Provides information and referrals of other agencies that may offer services for patients with ALS.
- Provide brief support and counseling to patients and families to help adjust and adapt to a changed lifestyle.
- Identify patient abuse or neglect and make contact with state investigating agency when appropriate - reports abuse as identified.
- Identify environmental impediments to patient progress through interview and review of patient records.
- Aid in special projects and other duties as assigned.
Core Qualifications
- Master's degree in Social Work
- Minimum 1 year of relevant experience required
- Valid State of Florida licensed clinical social worker
