Position Overview
The Clinical Liaison Support is a clinician and member of the sales team who partners with referral sources, discharge planners, providers, patients, and caregivers to coordinate the transition to home-based enteral nutrition and medical supply services. This role combines clinical knowledge, patient education, and care coordination to support positive patient outcomes and ensure a smooth start of care. This is an entry-level opportunity designed for clinicians interested in learning the enteral home care industry and growing within Aveanna Healthcare.
Referral & Care Coordination
- Meet regularly with referral sources including case managers, physicians, RNs, dietitians, and discharge planners to support referral processing and patient onboarding
- Complete documentation required for new patient referrals and coordinate with internal teams to ensure timely start of care
- Assist in resolving insurance, authorization, eligibility, and clinical documentation requests
- Support hospital discharge planning and transition-to-home coordination efforts
- Partner with sales team members to support growth and start-of-care goals
Clinical & Patient Support
- Provide education to patients and caregivers on enteral feeding pumps, supplies, and what to expect during service
- Troubleshoot basic pump, feeding, and tolerance concerns and communicate recommendations appropriately
- Follow up on new and existing patients to support continuity of care and patient satisfaction
- Assist with supply deliveries and patient education in the home as needed
- Collaborate with physicians, dietitians, nurses, and case managers regarding home feeding regimens and patient needs
Operations & Team Support
- Respond to patient care and revenue-related requests in a timely manner
- Coordinate communication between referral sources, patients, caregivers, and internal teams
- Aid in special projects and additional responsibilities as assigned
Required Qualifications
- Clinical background required (e.g., RD, LPN, DTR) with active credentials/licensure in the applicable state(s) of practice
- Associate or Bachelor’s degree from an accredited program
- Understanding of healthcare compliance and regulatory standards
- Ability and willingness to travel locally
- Strong communication, organization, and problem-solving skills
Preferred Qualifications
- 1–2 years of clinical experience preferred but not required
- Experience in homecare, enteral nutrition, DME, or discharge planning preferred
